Work Accident

An accident at work is defined as an unforeseen event that that results in a fatality, permanent disability or time lost from work occurred at the time and place of work, or path. This includes cases such as intoxication and the resulting acts caused by third parties, even outside the company's premises, when occurring in the working time. 

 

Accidents at work: those occurring in place and working time or path, where the employee is directly or indirectly under the control of the Company;

Accident in-itinere

Commuting accidents (in-itinere): those occurring during normal one-way journey back and forth between the home, the workplace and the usual place of meals.




Worked hours

Worked hours: Total of effectively hours worked in normal periods plus total of hours considered as overtime work in the reference period. It also includes the time spent with training actions and occupational  health services.




Days lost

the total time lost is the sum of the number of absent days (calendar days) resulting from occupational accidents occurring in the reference period, plus the number of days lost to accidents in the previous period which extended to the reference period without interruption. The lost time is measured from the day following the accident to the day right before the return to work. Temporary absences of less than one day, for medical treatment, are not considered as lost time.




Frequency index (Fr)

total of on-duty accidents, non-fatal with absence (TTD) and fatal, per million of worked hours in the reference period, with company workers.

 

 

 

 

 

Severity rate (Sr)

number of non working days due to non fatal on-duty accidents with absence (TTD), per million of worked hours by the company workers in the reference period.

 

High-Consequence On-Duty Accident

Accident resulting in injury(ies) from which the worker won't recover (eg.: amputation), may not recover fully or is not expected to recover fully within 6 months.

Recordable On-Duty Accident

The accidents are considered "Recordable", when they fit in the following typology:

  • Death;
  • Absence from work (TTI - Temporary Total Incapacity);
  • With Temporary Partial Incapacity (TPI) or Permanent Partial Incapacity (PPI);
  • Without absence, with use of non-prescription medication at prescription strength;
  • Without absence, with use of wound closing treatment, such as suture, staples, etc...;
  • Without absence, administering immunization vaccines;
  • Without absence, with use of devices with rigid stays/others designed to immobilization;
  • Without absence, with physical therapy treament;
  • Without absence, with loss of consciousness.
Near-Miss

A near-miss represents an incident where no injury or ill health occured, nor resulted in any health or environmental damages. The event could have been an accident if the circumstances had been slightly different or the activities hadn't been suspended.

Hazardous Situation

An hazardous situation reflects an event with potentiality to cause injuries, damages in the equipments and facilities, or other property damages.

High-consequence frequency rate (HFr)

Number of high-consequency on-duty accidents, per million of worked hours in the reference period, with company workers.

Fatal frequency rate (FFr)

Number of fatal on-duty accidents, per million of worked hours in the reference period, with company workers.

Recordable frequency rate (RFr)

Number of recordable on-duty accidents, per million of worked hours in the reference period, with company workers.
